Booster pads are designed to wear inside a protective undergarment, either a pullup or a brief with tabs. They don't have a waterproof backing so the fluid will flow through the pad into the garment underneath, which boosts the absorbency of the garment. They're not effective to wear inside regular underwear because the fluid will flow through to the underwear, making it wet.
